the above project address in accordance with the specifications listed below
subject to the terms and conditions of the original contract dated April
22, 2000, and will add on an additional ___three

date. The new substantial
completion date is now changed to July

Charges
1.)
Furnish and install approximately 34 square feet of Dorata
Beige #688822 (6”x 6”) ceramic tile set at 45 degrees angle with
Almond grout on walls between granite tops and the bottom of wall cabinets. Installation shall include ten (10) Corinzio C Border #660860 (4”x 4”), four (4) Corinzio Deco O/C #660861 (4”x 4”), six (6) Creta Mosaic #660870 (4”x 4”), and six (6) Corinzio B Deco #660862 (4”x 4”). Note: No tile on wall
at planning area is included.
Owner
agrees to pay contractor ONE THOUSAND TWO HUNDRED
SEVENTY DOLLARS ($1,270.00) for
labor and materials furnished hereunder, to be paid in full in advance.
IT
IS EXPRESSLY AGREED THAT NO STATEMENT, ARRANGEMENT, WARRANTY, OR UNDERSTANDING,
ORAL OR WRITTEN, EXPRESSED OR IMPLIED NOT CONTAINED HEREIN WILL BE RECOGNIZED
AND THAT THIS ADDENDUM CONSTITUTES THE ENTIRE AGREEMENT
By
signing this contract, Owner represents and warrants that he is the sole owner
of the aforesaid premises. This
addendum is binding on Contractor only when signed by or on behalf of Contractor
in the space provided below.
